EEAC's Sample Race, Ethnicity, and Gender Self-Identification Form

 

Federal regulations enforced by the Equal Employment Opportunity Commission (EEOC) and the Department of Labor’s Office of Federal Contract Compliance Programs (OFCCP) require that certain employers be able to identify the race, ethnicity and gender of each employee and, in the case of covered federal contractors, the race, ethnicity and gender of each applicant as well. 

These regulations do not restrict an employer’s ability to solicit and collect race, ethnicity and gender information from applicants or employees, nor do they specify how these data must be collected.  While providing applicants and employees the opportunity to self-identify race, ethnicity and gender is not technically required, it is EEOC’s and OFCCP’s “preferred” method of collecting this information, and the method used by virtually all employers.
